Physicians Practice reported today that starting January 1, 2027 nonpregnant adults ages 19 to 64 in Medicaid expansion groups must log 80 hours a month of work, school, community service or a work program or earn at least $580 to keep coverage, with the first sign of lapsed eligibility usually hitting the front desk at check in. The requirement stems from the One Big Beautiful Bill Act signed July 4, 2025 and the CMS interim final rule issued June 1, 2026 that requires states to implement by January 1, 2027. NEJM Perspective authors Nicole Huberfeld and Paul R. Shafer wrote October 22 that physicians will face new paperwork to document which patients qualify as medically frail and too sick to work, that lower Medicaid pay will follow from coverage losses, and that rural and safety net hospitals already under strain risk further financial pressure while estimates project up to 10 million people could lose coverage.
